Safety Plans

A written arrangement between caregivers and the agency that establishes how danger threats to child safety will be managed. A safety plan is a well thought-out approach, containing the most suitable people who take the necessary actions to control danger threats.

Step One:

Identify the Danger Threat(s) to child safety

Step Three:

Make sure the actions are specifically written so the responsible adults know exactly what needs to be done to protect the child

Case management monitors safety plan to ensure it is protecting the child

If the safety plan is not protecting the child, make changes!

The child will reside with the maternal grandmother. The maternal grandmother will take care of all the child’s basic needs; this is to include but not limited to feeding, bedtime routine, adhering to medical/dental appointments, and transportation to various appointments and activities. If the maternal grandmother is unable to care for the child, she will communicate with the maternal uncle. The maternal uncle will immediately come to the home to care for the child.

Homegirl Sally will facilitate supervised visitation with the mother. This includes but is not limited to transporting the children to and from the visitation. Homegirl Sally will plan directly with the mother for a specified day, time, and location of the visit. If the mother is inappropriate, belligerent, and/or intoxicated, Homegirl Sally will end the supervised visitation and immediately notify case management.

Sponsor Steve will visit the home randomly and unannounced once per day/night to check on the mother’s mental health status and ensure she is taking her prescribed medication as directed. Sponsor Steve will determine if the mother is overusing her medication and/or if the mother appears incapacitated; if this occurs, Sponsor Steve will immediately contact case management, contact the paternal grandmother, and remain in the home to care for the children.
