Measuring Outcomes

Using information obtained from service providers and through purposeful contacts, the case manager documents the progress and/or achievement of the case plan outcomes.

Selecting Excellent...

Document the dates of referrals and if applicable the dates of successful completion of services. Include information detailing how the parent has demonstrated behavioral changes from service providers, family support system, and children. 

Selecting Adequate...

Document the dates of referrals and as applicable the dates of unsuccessful discharge or successful completion of services. Include information detailing how the parent is beginning to demonstrate behavioral changes and/or how they may be resisting change; be sure to include statements from service providers, family support system, and children. 

Selecting Not Adequate...

Document the dates of referrals and if applicable the dates of unsuccessful discharges. Include information detailing how the parent is resisting case plan services; be sure to include statements from service providers, family support system, and children. 

Selecting No Progress...

Document all the dates of referrals and the dates of unsuccessful discharges. Include information detailing how the parent is resisting case plan services and their inability to enhance CPCs; be sure to include statements from service providers, family support system, and children.
