Behaviors:
Initiated contact (child, parent, worker)
Displayed affection and attachment (hug, kiss, smile, cry, avoid)
What did the child do? What did the parent(s) do?
What activities occurred during visit? (homework, played games, read books, snacks/meals, grooming)
What happened during the supervised visit?
Overall interaction: managed/redirected behaviors, attentive to child's needs (bathroom breaks, assisted child in vehicle)

Communication:
Non-Verbal communication (eye contact, proper voice tone, facial expressions, body language)
Topics of discussion included child's well-being (development, academic, peers, extracurricular activities)
What did the child say? What did the parent(s) say?
Clarified any concerns and/or questions the child may have in an age-appropriate manner and without bias or promises

Parent intoxicated
