
Home Visits



Important Details:
Separate interviews completed with all household members
How did the child appear? (hygiene, injuries, emotions: happy, sad, etc.)
What did the child say? What did the caregiver(s) say?
How does the child feel safe and comfortable in home? (activities shared with household, child’s culture, food preferences, religion, hair care)
What happened during the home visit?
Home observations: (home hazards, electricity/water, access to alcohol/chemicals, household members moving in/out)

Safety:
Completed walkthrough of the entire home (all rooms)
Assessed for child safety (age-appropriate sleeping, infant/toddler safeguards, water safety, alcohol & medication properly stored, etc.)
Provided an accurate case summary of the case circumstances, including the case type, participants, and any notable case facts
Reviewed Safety Plan (actions & responsible people) with entire family during the home visit
Clarified all concerns, issues, and/or questions the family may have had during the home visit

Well-being:
Reviewed frequency & quality of Family Time/Visitation (parent visitation & sibling visitation)
Discussed all past and upcoming medical, dental, mental health, and academic appointments
Reviewed the child’s medication (Rx, over-the-counter, vitamins)
Provided referral/service information (provider name, contact number, & address)
Reviewed Case Plan and Case Plan Service's progress
Assessed family dynamic (How does everyone in the home “get along”?)
Identified family roles (Who is responsible for what within the home?)
Verbalized the family strengths and needs (What is going well and what needs improvement?)
Promoted commitment and accountability amongst family members (Did you involve everyone with the case progress?)

Follow-ups:
Safety Plan
Are the Safety Plan Actions protecting the child? If not, what changes are needed?
Are the Safety Plan People completing the safety actions? If not, what changes are needed?
Have the Conditions for Return been met? If so, has a Progress Update & Safety Plan been completed?
Case Plan
Are the Case Plan Goal and Date still appropriate? If not, has a new Case Plan Worksheet been completed?
Are the services enhancing CPCs and improving Child's Needs? If not, what changes are needed?
Permanency
Is the Permanency Goal still appropriate? Discussed both the primary and concurrent goals?
Provided dates/times of upcoming events (Court Hearing, JR Hearing, Staffing, Visitation, Medical/Dental Appointment, or Academic event)
